The Double Diamond Ski Club is now accepting applications for the Windham Mountain Club Card program for 2011/2012 ski season. Remember you must be a member of the Double Diamond Ski Club to apply for a Windham Club Card Discount.

1. Send in your Double Diamond Ski Club Membership form and check (Included in this newsletter)
2. Print out the attached Windham Mountain Club Card Application form (Front & Back)
3. Fill out all the information on front for each person you are requesting a Windham Club Card for.
4. Sign the back of each form. Parents must sign if you are applying for a minor.
5. Send the Windham Mountain Club Card Application form (Front & Back) by scanning in the forms (Front & Back) and then e-mailing them to thomas.ladonne@us.ngrid.com
6. Once I receive your Windham Mountain Club Card Application form, I will check it against our membership database to confirm you are a member in good standing
7. I will then forward your Windham Mountain Club Card Application (with my signature) to Windham Mountain .
8. You can then pay Windham Mountain by:
• Mailing a check – to Windham Mountain Group Sales for the total amount, stating your name,
  and they will be able to match it up to the Windham Mountain Club Card Application form
  that I have already forwarded to them.
• Pay in person – to Windham Mountain Group Sales the next time you are at Windham ,
  and they will be able to match it up to the Windham Mountain Club Card Application form
  that I have already forwarded to them.

Please note – mailing the forms to me will delay processing and could jeopardize the application being received before the September 26th deadline
